Examinations and Eyeglasses

Lions clubs worldwide are dedicated to helping our community members obtain eye examinations and eyeglasses. Locally we collect hundreds of used eyeglasses each year for distribution to those in need and conduct local vision screenings in the area. We also provide assistance to those who are unable to afford the expense of these items, including vision therapy. 

Families in Dayton, Silver Springs, Virginia City and the surrounding area may download forms below and submit them to our club for consideration of assistance. Vision therapy forms are available upon request.
