Eyeglass Requests

The Sierra Vista Lions Club helps adults and children in our community who need eye exams and glasses but cannot afford them. Assistance is provided when there is a financial need.

 

Who Can Apply

You may qualify if:

• You live in the Sierra Vista area.

• You have a financial need.

• Your total household income is at or below the federal low-income level.

• You have not received glasses from the Sierra Vista Lions Club in the last two years.

How to Apply

1.   Download the Application Here

• If you can't download it, please email us at  svlionsclub@cox.net and we'll send you a copy.

2.  Complete the Application

• Fill out every section.

• Income includes YOUR income (wages, salaries, tips, commissions, etc.) PLUS any additional income in the household

• You MUST provide a working phone number and/or email.

• Incomplete forms will not be processed.

3.  Submit the Application

4.  Review & Approval Process

• The Lions Club will review your application.  

• You will be notified if you do not meet the requirements.

• If your application is approved, your information will be provided to the local optometrist our Lions club partners with.

 

6.  Scheduling

The optometrist’s office will contact you to set up an appointment.

 

7.  Your Appointment

• Arrive at least 10 minutes early to check in.

 

Policies

• No Show: If you miss your appointment without giving at least 24 hours’ notice, you may be removed from the program.

• Late Arrival: If you are 15 or more minutes late, you may have to reschedule.
