Marisol plans to attend Western Technical College's Associate of Science program with the intention to transfer to UW-LaCrosse to obtain a major in Nuclear Medicine on a pre-medical track. Marisol is interested in eventually working as a radiologist.


 


WHITEHALL LIONS CLUB HAS EXPANDED ITS REUBEN ADAMS SCHOLARSHIP - 2020

The Whitehall Lions Club announced it has expanded its Reuben Adams Scholarship.  The scholarship award is now $1,500.00.  It also has expanded its possible recipients to include not only High School Seniors but past graduates of Whitehall High School, persons who live in the Whitehall School District who are attending post-secondary education who have been home schooled and those who attended Whitehall High School, and who lived within the school boundaries while they received a GED. Pharmacists and pharmacy assistants may also apply.  Citizenship is no longer required.

Past winners may apply again.

Description of person being honored.

This scholarship is named in honor of Reuben J. Adams, M.D., and is intended to recognize a student entering the medical field. The medical field represents a broad aspect, including medical doctor, RN, CNA, nurse practitioner, physician’s assistant, pharmacist and pharmacist assistants and various rehab tech therapists. This list is not exhaustive, but serves as an example.The scholarship allows the student to define how his or her selection of a college (or tech school) curriculum demonstrates how this will have a positive impact on society. It is expected the applicant will complete an application and write a brief statement about their goals in life and how they intend to contribute to their chosen profession.

Dr. Adams was a man who committed himself to continuous learning and had a professional goal to help those in his care maintain an optimum level of mobility and well being according to their personal health condition. He was always well read and informed and used this knowledge in his medical practice. As a Lion, he was committed to supporting his community to the best of his ability in helping his fellow man.

Dr. Adams is missed by those who knew him for the care and help he gave to so many people in his career. This scholarship is intended to memorialize him for both his professional and community efforts.

SCHOLARSHIPS

The Whitehall Lions Club is pleased to award a scholarship in the amount of $1,500.00  to a student entering post-secondary education. The scholarship is intended for a student who is entering the medical field in any capacity. This includes, but is not limited to, medical doctor, RN, CNA, nurse practitioner, physician's assistant, rehab therapists and technical support. It would not include medical office work, reception or transcriptionist.

The scholarship is available at an accredit college, university or technical or community school or college.  You must have applied for admission.  Please list the names of any school below, whether accepted, declined or not heard.  List in order of preference.

DEADLINE:  Applications must be received in the High School Counselor's office by: May 10.  After completing this form, please return to the High School Counselor's office.

INSTRUCTIONS FOR APPLICANTS

1.   ELIGIBILITY AND REQUIREMENTS

(a)  Applicant must have applied to become a college, university, tech school or community college student. This scholarship is available to any applicant meeting the requirements.  The winner need not be a high school senior.

(b)  Applicant must be enrolled in an eligible school, intending to earn a degree in any medical field. 

(c)  Payment of the award will be submitted to the successful applicant after completion of the applicant's first semester in college and providing proof the applicant will be attending the second semester, assuming the student has a GPA of at least 3.0 out of a possible 4.0, or its equivalent.

(d)  Only applications with a documented high school transcript and ACT test scores attached will be considered.  The transcripts and the ACT test scores may be mailed separately, by policy of the high school.  Be sure to include the entire application form.

(e)  Applicant must be a Wisconsin resident.

(f) Current post high school students may apply.

(g) New this year is this scholarship is available to students who live in the Whitehall School District who have been home schooled or have obtained a GED.

2.   SELECTION PROCESS

(a) Initial judging will be conducted by the Lions Scholarship Committee.  The Club as a whole will choose the winner.  The club will not be told the names of applicants at time of determination.  They will be reviewing only the application.

3.   CRITERIA

The Lions Scholarship Committee will award the scholarship based on GPA, ACT, Class Ranking, Activities & Honors, Essays & Supplemental Credits. Supplemental Credits are those things which you have done in addition to the regular course curriculum to satisfy your own curiosity. Need will be considered.

(1)  In submitting this application, I authorize my high school principal or counselor to make available to the Whitehall Lions Club information concerning my academic records.

(2)  I hereby grant permission to the Whitehall Lions Club to release my name and award to the press.

FOR THOSE APPLYING WHO HAVE ATTENDED COLLEGE, PROVIDE THE SAME INFORMATION AS INDICATED ABOVE FOR YOUR CURRENT (OR LATEST) COLLEGE.

IN ADDITION TO THE ABOVE REQUIREMENTS, ON A SEPARATE SHEET OF PAPER:

  1.  List all of your unpaid service-learning activities that you have done from 9th grade through the present.  This is to include any and all organized unpaid volunteer work in the community NOT sponsored through a school organization.  For example:  Scouts, music, candy striper, etc.  This does not include sports activities unless you acted as a youth coach or some other form of volunteering.
  2. Please list any service learning/community service that you have done for individuals and/or neighbors who are not part of your immediate family.
  3. Submit a list of all the club/sports and activities with which you have been involved from 9th grade through the present.
  4. List your work experience since 9th grade through the present.
  5. Complete the goals in life essay.

Prepare a DOUBLE-SPACED TYPEWRITTEN essay of not less than 250 nor more than 500 words (not less than one nor more than two pages) and include the following:

Place your title of your essay along with your name at the top of the sheet.

(1)  How and when did you become interested in your field?

(2)  Explain your goals in life and how your chosen profession relates to the use of your talents and skills in a positive way.

(3)  How will your goals relate to the man after whom this scholarship is named?

(4)  Has anyone inspired you to choose this field? If so, who and how?

Emphasis would be on your goals and how they apply to your field of interest.

We are not interested in prose or descriptive writing. We want you to answer the specific questions.

STAPLE YOUR APPLICATION IN THE FOLLOWING ORDER from top to bottom: (You may submit via email after scanning your completed application with all attachments to the school counselor.)

How the money will be awarded:

  1.
