Sight and Hearing Program

Our service area includes McMinnville, Lafayette, and Dayton.  We are also temporarily (until further notice) covering Amity, Sheridan, and Willamina. If you do not reside within our service area, please complete the GET HELP FORM at OLSHF.org and you will be connected with your local Lions club or the District 36-O Task Force.

 

The McMinnville Lions Club works with specific providers of care (POC) to provide low/no-cost eye examinations, eyeglasses, and hearing aids. We can provide either vision or hearing services for an individual in any given fiscal year (which begins 1 July each year). You must decide which service you are most in need of before you apply. Eye examinations may only be granted every two years and hearing aids no sooner than five years since the last one was received.

 

You must attach documentation to your application that provides proof of your financial need for assistance. Following is a list of approved documents (Attach all that apply):

 

  • Last two pay stubs /or/ W-2 /or/ Tax return
  • Welfare benefit documentation /or/ Unemployment eligibility letter
  • Social Security documentation /or/ Disability documentation
  • Food stamp eligibility letter (NOT a copy of the OR Trail card)
  • A written statement from a trusted organization (YCCO/YCAP/DHS/Gospel Rescue Mission) verifying your lack of income.

 

Be sure to sign and date your application. Mail the completed packet to McMinnville Lions Club, PO Box 302, McMinnville, OR 97128.  Alternatively, you may scan the packet and attach it to an email addressed to maclions_sh@hotmail.com.

 

PATIENT CARE

If an examination reveals that surgery or special treatment is required (for eyes or ears) and your insurance won’t cover it, the Lions Club may be able to assist. There is a separate application process for these services.
