To start a membership application please do these steps:

1. Go to this website to download the application

2. Fill in the blanks

3. Print the completed form

4. Give the form to a member of the local Lions Club.

PS:  If you do not have a printer you may do the following.   1. After filling in the form you can right click on the form and "SAVE AS" to your system.  2. Then you can email the saved form to  georgetownlionsky@gmail.com
