What To Do If You Need Assistance with Eye Exam or Eye Glasses:

• Go to the Cherokee County Department of Human Resources to pick up an application for eye exam.  Fill out the paperwork with a DHR staff member.  Just tell them that you are there to complete an application for eye exam through the Centre Lions.
• The DHR office is located at 202 Hospital Avenue, Centre,  Alabama. Phone number is 256/927-1440.
• After you complete the paperwork, the DHR staff member will send the paperwork to our Sight Chairperson.
• The sight chairperson will review the request, either approve or deny the request depending on the circumstances.
• If request is approved, it will be passed on to the eye doctor.
• The doctor's office will then contact the applicant and make an appointment.  As the Lions Club, we do not make contact to the applicant or schedule appointments. 
• This process could  take 2 to 4 weeks.  Please be patient with us, we are volunteers and would like to help serve you with your eye exam and eyeglasses.
