Request for Assistance Form

All Requests for Sponsorship, Funding, and Equipment must be submitted in writing.  No other requests will be considered.  The Beaumont & District Lions Club Board of Directors prioritize and review all requests based on internally developed criteria. A response will be provided in a timely manner. 

Please fill out the following form as completely as possible. After completing the form you may submit in person at a regular meeting or send an e-mail to beaumontlions@gmail.com
