If you are a Corvallis area adult 18 years and older and meet Lions financial requirements, the Mid-Valley Lions club can help you. Pick up and complete an application for vision and hearing services at Corvallis Community Outreach, 865 NW Reiman, Corvallis.
Return your application to the address on the back of the form and include the required income verification documentation. If you do not live in Corvallis, contact the local Lions Club in your home town. Each Lions club provides services to their local residents.
Have used glasses or hearing aids to donate?
Drop them in the big yellow Lions drop box at 2330 NW Professional Drive, Corvallis (between Love, Inc. and United Way).
