Club Shirt and Vest Printing Order

Below is the order form for our club's shirt and vest printing. Please follow instructions on the order form. We need a total of 12 items before an order can be submitted to KulaThreads. When I2 items are ordered,  the order will be submitted and you will be informed. As soon as the orders are completed, you will contacted.

Polo shirt: Colors available Purple &Yellow (purple polo will have yellow logo, yellow will have purple logo)

Embroidered logo on polo          

 Cost: $15.71 Men’s(S-XL)  Men’s $17.80 (XXL) (Please place an “M” in front of shirt size for Men’s; ie. M-L for Men’s Large)

Cost: $15.71 Lady’s(S-XL) Lady’s $17.80 (XXL) (Please place an “L” in front of Ladies size ie. L-S for Lady’s Small)

Vest: (Screen printed: Big Leeward Oahu Lions logo on back of vest. See below)      Cost: $2.09

·      You must provide Vest. Please place a masking tape, with your first and last name,                  under the LCI patch
