HAVE YOU GOT THE BOTTLE?

What is it? It is a simple way of having your medical or medecinal details easily available in the event of the emergency services being called out so that they can give the appropriate care and attention. It is sponsored by Lions Clubs throughout the UK and supported by the Ambulance, Police, Fire and Rescue Services and the NHS Primary Care Trusts. 

How does it work? The information is kept on a simple form in a plastic bottle in the refrigerator.

Why the fridge?  Because if kept on a shelf in the door the bottle will never be pushed to the back and hidden.

What details are on the form?  Your name and description, your doctors name and surgery address, details of your medical condition, allergies and reaction to medication, location of your medication, names and addresses of next of kin and even details of any pets in your home. These details can also go to the hospital should you be taken there.

How do the emergency services know it’s there?  With each bottle you are given two labels, one to be placed on the inside of your front door and the other on the fridge.

What does this cost? Nothing the bottles are free from the Staines and Ashford Lions Club
