If you are a resident of Sevier County, TN, and are in need of vision services, please download and complete the Sevier County Lions Club Sight Assistance Program Application Form. Additionally, paper forms can be obtained from Eye Designs Optical, 1240 Fox Meadows Boulevard, Suite #2, Sevierville, TN, 37862. Completed forms are to be returned to the locations named on the bottom of the form. Returning form does not guarantee that services will be provided, but all requests will be considered and decisions made in a timely manner. Thank you
