 This event will be rain or shine! 

 Time: 12 P.M. – 6:00 P.M. (Setup 9 A.M.) 

 To be held on Deer Park Ave: From Strathmore Dr. to Dulittle St. 

Food Vendor Fee:* $200 per 10x15 space-Spaces will be assigned morning of event. Food Vendors must have S.C.D.O.H annual permit or send in attached S.C.D.O.H. temporary food vendor application. You will not be permitted to participate if you are not registered with Suffolk County D.O.H. Applications and fees must be in by 5/1/2017 EACH FOOD CATEGORY IS EXCLUSIVE!!! 

*Sausage and Peppers-Hamburgers and Hot Dogs are already taken. 

*You must leave the space exactly how you found it. NOTHING is to be left behind and you must cart all garbage away with you.

he/she will be allowed to exhibit on Deer Park Ave to be held on Sunday May 22nd 2016. If chosen to participate in the fair, the undersigned assigns, to release, discharge, indemnify and hold harmless its members, affiliates, associates, tenants, committee members thereof and from all claims, demands, action or cause of action, which may hereafter exist by reason of any damage, loss or injury which may be sustained by the undersigned in consequence of being allowed to participate in our North Babylon Community Festival and Street fair. This is a Rain or Shine Event. There will be no Refunds due to inclement weather.
