Message in a Bottle Scheme

HOW DOES THE SCHEME WORK?

As the name suggests, the participant’s personal information is kept in a bottle. The bottle is plastic with a push-fit lid and is big enough to hold the necessary information.

The bottle is kept in the fridge door, where the emergency services will expect to look for it in the event of being called to your home.

They will be alerted to your participation in the scheme by three green cross labels. One label is fixed to the inside of your front door (so that it is not visible from the outside) and the second is attached to the outside of your fridge door, the third is on the inside of the back door.

HOW MUCH WILL THE SCHEME COST ME?

There is no cost to the user. The basic cost of running the scheme is being funded by the Lions Club in your area

WHAT INFORMATION WILL BE NEEDED?

The information you need to provide is quite straightforward but, if you require assistance, you may like to ask a relative, neighbour or carer to help you complete the form. Please do not ask your Doctor to complete the form for you !

The information required on the form is:

  • Your full name and a brief physical description.
  • Your Doctor’s name, surgery address and telephone number.
  • A brief description of any medical conditions you are suffering from.

The Emergency Information Pack containing everything you need can be obtained from:

  • Any Pharmacy / Chemist.
  • Surgeries or your Doctor
  •  Local Hospitals
